I used to be an amazing Spanish 5 student, but haven’t thought about Spanish at all for the last 3 years. I want to brush up on it, but have thrown all my Spanish notes away. Where can I find a website that has all the grammar notes I need?

The following website is very good and correcting your errors by color coding them and explaining what you did wrong. It also provides translations and helps with writing in spanish.

    I can’t help with the grammar, but http://www.wordreference.com/ is an excellent dictionary to help with vocab, especially if you’re translating. It’s particularly good with colloquialisms and slang phrases you might have trouble with (search for the whole phrase if the individual words don’t seem to make sense).
